Housing Services Program Overview PATHS- Paths To Housing Stability

Who We Are: We provide emergency and transitional housing, case management, rental assistance, financial education, and advocacy and referral programs so that low-income, vulnerable populations can live in safety and make choices that promote family and individual self-sufficiency.

Our Vision: Our programs will enable individuals and families to stabilize their housing and improve their economic situations.

Division Background: Housing issues are consistently identified by low-income residents of Sonoma County as their most pressing need. Therefore, Community Action Partnership of Sonoma County has placed special emphasis on developing partnerships and programs to fill in vital gaps in the safety net in this area, including emergency and transitional housing, Fair Housing services for Sonoma County, tenant-landlord information and referral, and emergency assistance so families and individuals do not become homeless. The Agency also emphasizes partnerships and collaboration opportunities to develop and improve the system at large for stabilizing shelter and income for client families, and the community.

What We Do: Our programs include the Sloan House Emergency Shelter for Women, transitional shelters, Sonoma County Fair Housing, tenant-landlord mediation, case management, and homeless prevention programming.

  • Caring Communities: Case management for homeless families.
  • Sonoma County Fair Housing: Through 1-on-1 meetings, we help individuals and families to understand their rights under federal law (see website)
  • Rent Up: This program offers outreach and phone identification to assist individuals to advocate for themselves regarding tenant and landlord rights in Sonoma County, and landlord-tenant mediation services.
  • Financial FITness: This area encompasses small group education, accessing financial benefits, and building changes in lifestyles to instruct youth, individuals, and families about how to lead a financially stable life.
  • Shelter and Housing: Transitional housing for vulnerable women and families with children who lack housing stability.
  • HCA Fund: This is a rental assistance program to keep vulnerable families in their homes and apartments and prevent them from becoming homeless.
