Community Transformation Project (CTP)
The Community Transformation Project aligns with Health Action, Sonoma County’s collaborative effort to improve the health and health equity of all residents, and is funded by the Center for Disease Control and Prevention’s (CDC) Community Transformation Grant (CTG) to improve the health of small communities across the nation.
The goals of CTP – South Santa Rosa is to increase the number of South Santa Rosa residents access to healthy and safe physical environments. We will achieve that goal by educating the community residents about comprehensive approaches to improve community design to enhance walking and biking and bicycling in South Santa Rosa. The project is currently focused on collaboration with the Moorland Neighborhood Advisory Team and the County of Sonoma to support resident-led efforts improving community health and safety.
